So possibly a train and engineer problem. I would have thought the train would have made several practice runs before they started carrying passengers. That would be the logical thing to do.

Not sure practice runs would have helped... if it was speeding.. the train was running 30 minutes late and it's looking like the engineer was trying to make up time..
